With our recent name change from Triad Consultants to Vigilant Employment Screening, Inc., we have revamped the Background Check Authorization.  There have also been a number of State Laws passed and/or amended, and such changes have been incorporated in our revised Background Check Authorization.  

You can access the new Background Check Authorization form below.  The applicant information can be directly entered into the appropriate fields on the form.  You can then print it out for ink signature and optionally save a copy for your files.  

Depending on your browser settings, the Background Check Authorization will be downloaded to your "Downloads" folder or it will appear as a second web page where you can then use it and save it to your computer for future use.
